What to bring to your first appointment
Please arrive ten minutes early on your first appointment to leave time to fill out some forms. To make things go smoother, please bring along a list of medications you take, your insurance information, and the name and phone number for your family doctor.
Financial policy
Payment is due at the time of treatment. We accept major credit cards, cash, and debit, and we offer payment plans to assist in paying for your treatment.
Insurance
We will assist you with filing claims with your insurer. Please note that you are responsible for the full cost of your treatment, including any amounts your insurer does not cover.
